Evaluation Plan
Pause Well has an internal evaluation process to help assess its value and effectiveness. The plan provides feedback for comparison to other health promotion programs, eliminating weak components and gathering information about the program. All findings will be shared for the benefit of stakeholders and used to improve future Pause Well programs.
Insights into Outcomes Achieved
To gain insights into whether the program achieved its outcome objectives and goals, Pause Well conducts evaluation surveys to assess program implementation, program effectiveness, and participant outcome goal achievement from baseline to program completion (end of 4 months), six months, and 1 year after completion. All surveys are self-reported questionnaires using Likert scales where appropriate. Participants are asked to set an overall wellness goal on the first day of the program and track their progress. Participants are also asked to note their biomarkers from their last wellness check and look (and note) for future bloodwork changes. All evaluation surveys will be given at launch, completion of 4-week program, 6 months, and 12 months. Objectives include:

80% of participants will complete the survey on the first and last of the program.

50% of participants will complete the 6-month follow-up surveys via FB and email.

50% complete the 12 month surveys via the FB platform and email.

The leader informs 90% of healthcare providers and managers of the evaluation results.

The leader continues to improve the program’s goals, objectives, activities, presentation, and materials based on evaluation feedback for future implementations.
